Improve the health and wellbeing of women and families in Wisconsin by purchasing a Wisconsin Women's Health Foundation license plate. Your $25 donation will be re-invested in communities all over Wisconsin:

• 50% will support Wisconsin Women's Health Foundation outreach and education programs that reach thousands of Wisconsin women and families each year.
• 50% will support Community Health Grants, which provide funding for community-based women's health programs in Wisconsin.

Your donation can be made "in honor" or "in memory" of someone you love. Information will be enclosed along with your new Wisconsin Women's Health Foundation license plate. There's no need to wait until your registration is due to apply! Complete the application today, and you'll simply pay the one-time original issuance fee of $15, plus the $25 donation (note: if your current plates expire within 3 months, you'll also need to pay your regular registration fee). Your specialty plates will be mailed to you. Each year you renew your Wisconsin Women's Health Foundation license plate, the Foundation will continue to receive a $25 donation.
